The NCMHCE content outline (NBCC, current edition) defines the entire scope of what's testable on the exam. Five clinical work behavior domains, each tied to the activities a competent clinical mental health counselor performs in practice. Below: what each domain covers and what to study within it.
Domain 1: Professional Practice and Ethics
The ACA Code of Ethics, NBCC standards, mandated reporting, informed consent, confidentiality limits, dual relationships, and scope of practice. This domain shows up in nearly every case — ethics is not a separate section; it's embedded throughout decision-making.
Domain 2: Intake, Assessment, and Diagnosis
Clinical interviewing, mental status exam, biopsychosocial assessment, suicide and homicide risk assessment, DSM-5-TR diagnostic criteria, differential diagnosis, and use of standardized assessment instruments. The largest single content area on most exam administrations.
Domain 3: Areas of Clinical Focus
Crisis intervention, trauma-informed care, substance use, grief and loss, suicidality, intimate partner violence, abuse and neglect, and culturally responsive care. Cases pull from this domain whenever a presenting concern requires a specialized response.
Domain 4: Treatment Planning
Goal setting (measurable, achievable, time-bound), evidence-based treatment selection, collaborative case conceptualization, referral and consultation, and treatment plan revision based on progress monitoring.
Domain 5: Counseling Skills and Interventions
Therapeutic alliance, microskills, specific intervention modalities (CBT, DBT, MI, prolonged exposure, EMDR, family systems), group counseling, and termination. The intervention sections of each case score most heavily here.
